What is a Congregate Living Health Facility?
A Congregate Living Health Facility (CLHF) is a residential inpatient care facility, which provides the following basic services: medical supervision, 24-hour skilled nursing and supportive care, pharmacy, dietary, social, and recreational activities. CLHFs provide skilled nursing care on a recurring, intermittent, extended, or continuous basis. The care is generally less intense than that provided in general acute care hospitals, but more intense than that provided in skilled nursing facilities. Preferred Care Facilities specialize in services for individuals who are mentally alert, physically disabled who may be ventilator dependent and individuals who are catastrophically and severely disabled through trauma or degenerative neurologic illness, where it has been determined that active rehabilitation would be beneficial.
Our Story
We currently have two facilities to serve the Los Angeles area, one in Northridge and another in Sunland. We believe in an interdisciplinary approach to healthcare. Our facilities are led by a group of pulmonologists and includes various physicians, nurses, case managers, physical, occupational, speech and respiratory therapists, psychologists, pastoral care staff and registered dietitians.
We work with most payors, such as HMOs, PPOs, Worker’s Comp, private insurances and private pay.
